Referred by Lois at Oak Street Health 812-554-4373. I spoke to patient and he is interested in ongoing assistance with meal preparation, running errands, and light housekeeping. Patient would benefit from having someone to spend time with and he would like to have someone who would participate in community activities with him. Patient is unable to prepare his own meals. This is an ongoing need. Patient is an adult with Autism.
